I think I remember this happening on my Mac in C5.
unfortunately sometimes, with multiple sounds loaded into HSO, sometimes it loses the slots.
Always save your preset!
I also always make double sure to name my channels as the instrument I load, so I remember what to load back in.
Just protect yourself.
